The American Academy of Pediatrics Project Grants Database

This database archives current and previously funded community-based grant projects, including the CATCH Program and the Healthy Tomorrows Partnership for Children Program. The database is searchable by 7 major categories: target population, health topic, state/territory, project activity, AAP program, AAP district, and project year. Search results provide the project description, goals, keywords, and location.

The Coalition of Community Foundations for Youth (CCFY) is a network of over 200 community foundations in communities across the United States dedicated to securing improved conditions for children, youth and families.

Community-Campus Partnerships for Health: http://www.education.com/partner/articles/cchp/
The Center for Health Professions at the University of California, San Francisco, has developed the Community-Campus Partnerships for Health (CCPH) Program. It is a nonprofit organization that promotes health through partnerships between communities and higher educational institutions. The site features strategies for working in partnerships with communities, service learning projects in the education of health professionals, and resources for working with communities.

The Health Resources and Services Administration (HRSA) Find a Health Center helps you locate a federally-funded health center in your community.
