AZ—KidsCare Reach Out, Tucson, AZ: $10,000
Collaborative Partners Service Area Program Objectives
El Rio Health Center will partner with Pima County Access Program, including all community-based clinics in Pima County, five hospitals in the county, and the Pima County Medical Society and University Physicians, Inc.; AAP Arizona Chapter; Pima County and Maricopa County Pediatric Societies
Pima County with hub in Tucson; outreach to SCHIP eligible in private pediatricians’ offices
Expand awareness and knowledge of eligibility for KidsCare in private pediatricians; assist potentially eligible persons in private pediatrician’s office to apply for KidsCare
CO—Denver Covering Kids, Denver, CO: $10,000
Collaborative Partners Service Area Program Objectives
Denver Covering Kids; Denver Public Schools; Blockbuster and local movie theatres; Fenix project at Mi Casa; Child Health Plan Plus; Colorado Covering Kids
Uninsured adolescents in Denver County through Denver Public Schools
Increase SCHIP enrollment of Denver County adolescents by 500; establish advisory committee and lead focus groups to develop appropriate marketing messages; conduct parent informational seminars; develop marketing material and coordinate peer-to-peer referral program
FL—Haitian-American SCHIP Access Program,Miami, FL: $10,000
Collaborative Partners Service Area Program Objectives
University of Miami Pediatric Mobile Clinic will partner with Jackson Memorial Hospital Nursery; Toussaint L’Ouverture Elementary School; Jefferson Reaves; Cecile Day Care Center
Miami Dade County; uninsured Haitian-American children living at or below 200% of the Federal Poverty Level
Identify uninsured Haitian-American infants/children who qualify for KidCare; identify barriers to enrollment for qualifying Haitian-American children; facilitate enrollment process into KidCare; provide healthcare service for non-qualifying
FL—REACH OUT, St. Petersburg, FL: $10,000
Collaborative Partners Service Area Program Objectives
Lawton Chiles Children and Family (LCCF) Health Care Center will collaborate with Manatee KidCare Coalition; Whole Child Project; health Department, Childrens Services; Glen’s Corp; Girls/Boys club; Lamaz Class
Manatee County; uninsured children seen at the LCCF, including large population of migrant workers
Provide outreach to enroll uninsured children into SCHIP; establish contact with at least 90% of uninsured families; decrease by 50% the number of uninsured at LCCF health center
IL—Children’s ER-based KidCare and Medical Home Initiative, Chicago, IL: $10,000
Collaborative Partners Service Area Program Objectives
Children’s Memorial Hospital will collaborate with Community-based Children’s Health Consortium; Chicago Community Health services; Erie Family Health Center; Infant Welfare Society; Near North Health Service Corporation
Uninsured children utilizing the hospital’s emergency department
Provide education and enrollment assistance to eligibles utilizing the ED; link appropriate ED patients with community health centers for KidCare enrollment and care; provide education on primary care and medical home to all targeted patients; establish baseline data on number of patients within target population who have no regular source of care.
IL—Child Health Access Project, Chicago, IL: $5,000
Collaborative Partners Service Area Program Objectives
Access Community Health Network will collaborate with Fojo Management Inc., managing community relations for McDonalds
South Lawndale area of Chicago; uninsured children of employees and patrons of McDonalds restaurants
Provide information, training, & eligibility screening for crew at 5 McDonalds restaurants regarding KidCare and community healthcare; provide outreach events at local McDonalds
IA—Engaging the Physician Office in SCHIP – Impact on Enrollment, Cedar Rapids, IA: $10,000
Collaborative Partners Service Area Program Objectives
Healthy Linn Care Network, HACAP, Family & Child Health Alliance, Free Medical Clinic at Jane Boyd Community House
Linn County with Cedar Rapids as a hub; every SCHIP eligible within a primary care setting
Increase provider/staff SCHIP awareness & knowledge; increase SCHIP point-of-service outreach; increase SCHIP enrollment within each practice by 50%
ND—Physician/Clinic Outreach Initiative, Grand Forks, ND: $9,963
Collaborative Partners Service Area Program Objectives
AAP North Dakota Chapter; North Dakota Department of Human Services; North Dakota Covering Kids; clinics employing pediatricians; North Dakota Medical Association
Uninsured children of North Dakota through 77 pediatricians statewide
Educate pediatricians regarding SCHIP eligibility and practice needs; provide clinic staff training and link clinics to Human Services for follow-up; provide materials to support enrollment and outreach
SC—Eau Claire Cooperative Health Center Outreach Program, Columbia, SC: $8,000
Collaborative Partners Service Area Program Objectives
Eau Claire Cooperative Health Center will collaborate with Hyatt Park Elementary School, site of school clinic; Columbia Urban League, for outreach workers; Eau Claire Community Council, assistance in promotion
Eau Claire section of Columbia; low-income, minority SCHIP eligible children concentrated at Hyatt Park Elementary School
Assess eligibility of low-income children through door-to-door campaign; increase SCHIP enrollment and offer medical home; provide real-time services in neighborhood school-based clinic
TX—Every Child A Healthy Child CHIP Outreach, Education & Enrollment Project, McAllen, TX: $9,780
Collaborative Partners Service Area Program Objectives
Children’s Defense Fund – Rio Grande Valley; Coalition for Valley Families; Lacar Kids’ Clinic
Children ages birth to 18 years of age living in the lower Rio Grande Valley of Texas
Develop a CHIP education and enrollment train the trainer curriculum; Increase by 300% the number of CHIP service sites; Increase by 100% the current rate of enrolled children in CHIP.
TX—TexCare Partnership of the Highland Lakes: $5,700
Collaborative Partners Service Area Program Objectives
Outreach Health Services of Burnet County; Seton Highland Lakes Hospital; The Seriff Foundation; Sandra Thomas, MD
Uninsured children in Burnet County, Texas
Solicit and educate volunteers to provide outreach services; Provide a variety of events and materials for distribution to promote SCHIP
VA—Medical Community Outreach Project, Richmond, VA: $10,000

Increase number of children enrolled in SCHIP; increase consumer knowledge of SCHIP; increase physician awareness and outreach and enrollment in partnership with physician practices; establish outreach systems at local community care settings using medical students as volunteer outreach workers
WA—Creating Greater Clinic Involvement in Signing Up Children for Medical Insurance, Longview, WA: $10,000
Collaborative Partners Service Area Program Objectives
Collaborative partners are eight clinics in Cowlitz County; Parent’s Place; Children’s Community Resources
Cowlitz County uninsured children utilizing clinics, health department, and hospitals
Develop procedures for outreach and enrollment of uninsured children for office staff; enrollment of 250 additional children; convene partners to gather data, train clinic office personnel in enrollment, develop patient literature to assist in enrollment, obtain additional funding
WA—Crisis Nursery Outreach, Seattle, WA: $10,000
Collaborative Partners Service Area Program Objectives
kids.health.2001; Washington Health Foundation; Healthy Mothers Healthy Babies; Department of Social and Health Services; University of Washington
Uninsured children utilizing services of crisis nurseries throughout the state of Washington
Establish relationship between crisis nurseries and Washington Chapter AAP; provide on-site training to nursery staff regarding SCHIP; provide information to families utilizing the nurseries
