Community Foundations
Many community foundations provide program assistance in areas
such as strategic planning, community needs assessment, and program
evaluation. Community foundations often fund local health-related
initiatives and also can suggest key business and community people

Conversion Foundations
A conversion foundation is a new health foundation that is created
when a nonprofit hospital, health maintenance organization (HMO),
or health insurance plan sells or otherwise transfers its assets
to another corporation. Most conversion foundations primarily
fund health-related projects that are targeted toward a defined
geographic area. In 2003, Grantmakers in Health conducted a survey

Cooperating Collections of the Foundation Center
The Foundation Center is an independent national organization
that provides information on foundation and corporate giving and
publishes directories of foundations and other grant makers for
those seeking grants. The cooperating collections of the Foundation
Center are institutions such as libraries, community foundations,
and other non-profit agencies that make these directories and

Academy chapters are organized groups of pediatricians and other
health care professionals working to achieve AAP goals in their
communities. Chapters are the channels of representation for individual

The Medicaid program is the largest health insurance program for
children in the country. The Medicaid Director administers the
