Description

The EQIPP Social Health and Early Childhood Well-being course uses early relational health to foster family resilience and assess social needs impacts on general development and social-emotional health. Pediatric primary care practices can implement practice transformation strategies to improve screening, counseling, referral/linkage, and follow up rates with families during well child checks (WCC) in early childhood.

By the end of this course, the learner will be familiar with how to:

  • Elicit and address the interests and concerns of the family.
  • Learn from and build on the strengths of the family.
  • Screen for perinatal depression of the caregiver using a validated tool and take appropriate action when the screening result is positive or concerns about depression are present
  • Assess and address social drivers of health that emerge from the family’s and community’s circumstances, and which affect the child’s and family’s health in positive and negative ways.
  • Screen for social emotional developmental using a validated tool and take appropriate action when the screening result is positive or concerns are present
  • Establish follow-up plans to address identified interests, concerns, and social health needs or positive screens/assessments
