This section has been developed for all of the health care professionals involved in providing health care for children or teens in foster care, including pediatricians, family practice physicians, nurse practitioners, and others. The resources that follow are intended to assist all health care professionals in improving their primary care office setting so that each child or teen in foster care receives health care in a medical home.

We have developed information for all health care professionals – those who are just starting to see children and teens in foster care; those who have more than a beginner’s interest in providing health care for children and teens in foster care, but are not an expert or specialist in the field; and those who see a lot of children and teens in foster care, have a long-term interest in this topic, or specialize in treating children and teens in foster care.

Included on this section are forms for use in your practice or organization, tip sheets, educational materials for families, and links to pertinent Web sites to get more information.

pdf Back to Sleep for Babies in Foster Care:
Every Time, with Every Caregiver

For judges or caseworkers to provide to all caregivers involved in a baby’s care (including foster parents and birth parents) to educate them about safe sleep, and pledge to follow the proper safe sleep practices.

of Information

To be completed by a patient, or the guardian of a patient, who is allowing a practice to obtain specified confidential records.

To be completed by a patient, or the guardian of a patient, who is consenting to the release of confidential records or information, to a specified practice.

pdf Health Information Form
For use by health care professionals to assist in better assessing and monitoring children and teens in foster care. Such documentation will ensure that children in foster care receive the best possible care.

pdf Health Summary Form
To be completed (or updated) by the physician/provider at the conclusion of every visit, and given to the child’s or teen’s foster parent(s) and caseworker as a complete summary of the most important health issues, treatment plans, and upcoming appointments.
