Maine Chapter

This AAP Maine Chapter pilot project will involve 4-5 geographically diverse pediatric practices working together as a collaborative with pediatric endocrine, nutrition, and psychology consultation to develop a Chronic Care Model including consistent screening and evaluation methods, patient empowerment and behavior change methodologies, and community integrated care plans for the management and secondary prevention of obese and overweight children (over 85-95% BME for age). Anticipated patient goals include BMI percentile reduction, increased daily physical activity, lipid profile improvement, and adult onset diabetes prevention. System goals include developing a closer collaborative relationship between Maine AAP and the State Bureau of Health in reaching children's Healthy Maine 2010 objectives; developing local community collaboratives looking at primary prevention of childhood obesity; practical program evaluation process with collaborator Maine-Harvard Prevention Research Center; and hopeful spread of this pilot to a larger statewide collaborative of family practice/pediatric offices through a subsequent grant (eg. Maine Health Access Foundation).
