The Tulalip Tribes is a federally recognized Indian Tribe located on the Tulalip Reservation in the mid-Puget Sound area. The Treaty of Point Elliot established the reservation to provide a permanent home for the Snohomish, Snoqualmie, Skagit, Suiattle, Samish and Stillaguamish Tribes and allied bands living in the region.

Our practice is part of a multi-disciplinary group on an Indian Reservation that serves only Native Americans. There are 3 family medicine doctors, 1 pediatrician, and 1 internal medicine doctor. We have a mix of privately insured, publicly insured and Indian health insured patients. The Tulalip Tribal Health Clinic provides a variety of primary care services including a dental, optometry, and alternative modalities. Our dental clinic has 3 general dentists, 1 orthodontist and 1 pediatric dentist. The dental clinic also provides general anesthesia once a month for the oral needs of the kids in the tribe who are young, uncooperative or have special needs.

Given the high level of poverty and lack of fluoridation of the drinking water and the easy access to sugary drinks in our country, the children of our community are at very high risk for developing dental caries. Our infants are also at high risk for being low birth weight and premature, which both can place these babies at risk for enamel defects and high caries risk. The children will often seek primary care providers first due to childhood illnesses before they seek the dentist.

We are referring the kids at risk to the dental clinic as soon as there is a tooth in the oral cavity or by their 1st birthday. Patients and families can also seek care at the dental clinic without any referral. Our site currently applies fluoride varnish in the dental clinic. We have programs where kids who are at ECAP or Head Start get oral exams by the pediatric dentist, and get fluoride varnish applied at that one visit. Cards given to parents regarding their DMFT (decayed, missing, filled teeth) score and asked to have their child brought into the dental office for routine care or urgent care. Sealants are placed in the first and third grade once a year by the dental clinic doctors and staff.

Rama Oskouian, DMD, is a graduate of the University of Pennsylvania's, School of Dental Medicine where she graduated in 2000. Enjoying her time on the east coast, she completed a fellowship in pediatric dentistry at the prestigious Children's Hospital of Philadelphia. Following her fellowship, she completed two years of residency training in pediatric dentistry at Cincinnati Children's Hospital Medical Center.In 2003 she enrolled in a two year fellowship in pediatric dentistry at the University of Washington. In between training new dentists on how to be pediatric dentists, she earned a Master of Public Health degree with a specialty in Maternal Child Health in the fall of 2005 and earned a certificate in the Teaching Scholars program.

She has also been actively involved in training Pediatricians, Pediatric and Family medicine residents and medical students about pediatric oral health. Dr Oskouian has given multiple presentations at national, regional and local meetings and local hospitals. She is passionate about helping children access dental services, and training primary care providers in helping provide preventive services. Dr Oskouian also has extensive experience in the use of fluoride varnish and in the education of dental and non-dental personnel in doing oral health assessments and recognizing early signs of dental decay through the ABCD program in Washington State.

Dr Rama is also thrilled to be the Washington State Dental Director for the Special Olympics, where she screens athletes and educates them and their families about oral health. She also practices part-time at the Tulalip Indian Health Clinic. Dr Rama serves on the AAP's Oral Health Intiative Steering Committee and is also active with the AAP training of medical professionals in oral health and helping reduce the caries risk in population.
 
Training Day    
The goal of our training day was to provide oral health information to both the providers and the staff of our clinic in order to increase their knowledge of why oral health is important and how dental caries is preventable disease. Most clinic staff are Tulalip tribal members. Our hope is that the word would spread throughout the community and we can start reducing the caries rates among the children who then grow up to be active community members.

Our day started out with an offering of low or no cariogenic snacks. After discussing the services that are offered through our dental clinic, Dr Rama Oskouian gave a wonderful power point presentation. We had an active question and answer session. Dr Oskouian also brought samples of xylitol gum, fluoridated drinking water, and toothpaste. We ended the session with a raffle for several sonic toothbrushes. We have received a lot of positive feedback from those who attended the session.
   
Next Steps    
We plan to continue the education of the community and the staff. The community has requested further presentations by Dr Oskouian. We hope to reach out to the day cares, preschools, and other agencies taking care of tribal children. We have encouraged the WIC program to refer children with dental needs and for their routine care. We have instituted an "in-house" referral system to help track children in need of preventative services and to stress the importance to families.In addition, we have added prenatal dental visits to our referral list.
