PROS Child Behavior Study
PROTOCOL OVERVIEW

Participation in the PROS Child Behavior Study involves the following basic steps:

1. Identify an Office Coordinator

Identify an on-site office coordinator(s) who will manage the day-to-day study activities and who will communicate with PROS Central.

2. Determine the Eligibility of All Patients

3. Enroll Consecutive Study-Eligible Patients

Record information for all study-eligible patients in enrollment log.

Complete Clinician Visit Questionnaire (10 brief questions on all children and 25 brief questions on children with a clinician-identified psychosocial problem).

Parent/Caregiver completes the Parent/Caregiver Questionnaire.*

Mail enrollment logs and all clinician and parent/caregiver forms in packs of 10 to PROS Central.

Send six-month Follow-Up Questionnaires to parents/caregivers of patients randomly selected for follow-up (approximately 3-5 patients per clinician).

* Spanish-language parent/caregiver forms available.


Reimbursement
Practices will be reimbursed $150 at the completion of initial data collection and an additional $150 at the completion of the follow-up component.
