Children's Health Survey for Asthma (CHSA) | |  |
Thank you for your interest in the American Academy of Pediatrics' (AAP) Children's Health Survey for Asthma (CHSA). The AAP is a not-for-profit organization committed to the attainment of optimal physical, mental and social health and well-being for all infants, children, adolescents, and young adults.
Developed for parents of children ages 5-12 years with asthma, the CHSA addresses aspects of physical, social and emotional well being for the child and the family. While there is no charge to you for the CHSA or its use in non-commercial research or practice applications, we require that you complete the online "User Registration and Copyright Agreement" process. The information you provide will help us learn more about how the instrument is being used and how it can be improved in the future.
Once you complete the "User Registration and Copyright Agreement" form, you will gain access to the following materials:
