Developed for parents of children ages 5-12 years with asthma, the CHSA addresses aspects of physical, social and emotional well being for the child and the family. The CHSA-C is an interviewer-administered (children ages 7-16 years) or self-completed (children ages 10-16 years) measure for children with asthma. The CHSA-C can be used alone or as a companion to the parent-completed CHSA.

While there is no charge to you for the CHSA or its use in non-commercial research or practice applications, we require that you complete the online "User Registration and Copyright Agreement" process. The information you provide will help us learn more about how the instrument is being used and how it can be improved in the future.
