The most common transition is the transfer of care responsibilities. A multidisciplinary care team, providing care for a medically complex patient, will usually transfer responsibilities in parallel (physician to physician, nurse to nurse, etc) and asynchronously. This session focused on the process and outcomes and also provided an opportunity for actual case discussion.
