Calvin C. J. Sia Community Pediatrics Medical Home Leadership and Advocacy Award

Named in honor of Calvin C. J. Sia, MD, FAAP, the Calvin C. J. Sia Community Pediatrics Medical Home Leadership and Advocacy Award recognizes pediatricians who have demonstrated clinical excellence, community action, and advocacy for children. This award honors those pediatricians who have exemplified leadership and excellence by advocating for and providing a medical home for all children, especially children with special health care needs. The Calvin C. J. Sia award will be given to one member of the American Academy of Pediatrics each year.

The COCP is currently accepting nominations for the Calvin CJ Sia Community Pediatrics Medical Home Leadership & Advocacy Award for 2009. For more information see the call for nominations and nomination form.

DEADLINE EXTENDED:

***All nominations are due by 5pm (CT) on Monday, May 11, 2009 .***

The award is presented to community pediatricians who epitomize the field of community pediatrics through the following activities:

  • Implements AAP policy related to community pediatrics and the provision of medical homes (e.g., Medical Home, The Pediatrician's Role in Community Pediatrics) into practice and action.
  • Exemplifies services, action, and advocacy that move beyond one child to providing a medical home for all children in the community, and moves beyond the current standard of practice to improved models of pediatric care delivery.
  • Provides culturally competent care through the recognition of cultural, spiritual, economic, environmental, and political forces that have an impact on the health of children.
  • Partners with families in the provision of medical homes for their children.
  • Demonstrates a commitment to the role and duty of the pediatrician in the community as a professional, leader, and advocate to improve child health outcomes.
  • Exemplifies family-centered practices and the partnership of families in the provision of a primary care medical home for all children.
  • Demonstrates a commitment to continuous improvement in the delivery of care in a medical home.
  • Exemplifies clinical excellence in community pediatrics.
