Prerequisites for Acceptance:

  • 1) Universal AMSSM Sports Medicine Fellowship Application (http://www.amssm.org), 2) Personal Statement, 3) Dean's letter, 4) 3 letters of recommendation (including one letter from residency program director), 5) Medical school transcripts, 6) USMLE transcripts Part 1,2,3 scores with statement on number of attempts, 7) If applicant has rotated through a Kaiser Permanente facility, an optional letter from a Kaiser Permanente attending physician is an added bonus.

Additional Comments: Applicants should have completed or be in the process of completing a residency in Family Medicine, Internal Medicine, Emergency Medicine or Pediatrics at the time of application and should be Board eligible or Board certified in the respective discipline.
