Qualifications and Requirements

| 1. | Physicians
not eligible for Fellowship in the Academy, whose practice or research includes
the interfacility transport of pediatric patients.
| Applicants
must submit:
Completed
application form and any necessary fees.

| 2.
| Nurses
whose clinical practice, research, or administrative responsibilities include
the interfacility transport of pediatric patients. | Applicants
must submit:
Completed
application form and any necessary fees
and
a
copy of their nursing license.
|
| 3. |
Respiratory
Care Practitioners whose clinical practice, research, or administrative
responsibilities include the interfacility transport of pediatric patients. | Applicants
must submit:
Completed application form and any necessary fees
and
a copy of their state license (or documentation from a supervisor that licensure
is not available in their state) and proof of certification or registration by
the National Board of Respiratory Care.

| Paramedics
(EMT-P's) whose clinical practice, research, or administrative responsibilities
include the interfacility transport of pediatric patients. | Applicants
must submit:
Completed application form and any necessary fees
and
a copy of licensure by their state or by the National Registry of EMT-P's.

| Physician
Assistants (PAs) whose clinical practice, research, or administrative
responsibilities include the interfacility transport of pediatric patients. | Applicants
must submit:
Completed application form and any necessary fees
and
a
copy of their state license.
