Program Description

Problem: Childhood obesity is a serious problem in the United States, putting not only children at risk for poor health but creating a generation of unhealthy adults. Number of children affected: The prevalence of obesity for children and adolescents aged 2-19 years was 19.7% and affected about 14.7 million children and adolescents in 2017-2020[1]. Obesity threatens the health of today's children to such an extent that they may, for the first time in US history, have a shorter lifespan than their parents.[2] Primary setting: It will be a multi-setting clinic-based intervention with community partnership to address the most burning health issue of today’s time. The AAP recommends counseling parents at every well-child check on lifestyle goals as a part of obesity prevention initiatives.[3] “Let’s go 5-2-1-0”was originally used by the Maine Center for Public Health's Maine Youth Overweight Collaborative[4]. It stands for:  5—eat 5 fruits and vegetables per day  2—limit screen-time to ≤2 hours/day  1—≥1 hour of physical activity  0—avoid sugary drinks Project goal: This project aims at endorsing healthy “5-2-1-0” habits to all the children aged 2-9 years old encountered during well child visits and thus prevent obesity in early ages. · Evaluate baseline habit choices using AAP-Healthy Habits questionnaire · Make clinics 5-2-1-0 friendly via community partnership · Link families for resources and food agencies via partnership · Engage local youth leaders and develop a multimedia method for 5-2-1-0 message . Identify most effective educational means to provide counseling regarding 5-2-1-0 goals · Evaluate the effect of educational interventions Challenges: There are several factors which can affect the rates of attainment of 5-2-1-0 goals- income, race, parent education, family dynamics, household composition and demographical factors to name a few. Physicians also face unique barriers to utilize this tool. It is an understatement to quote that this tool is underutilized in the community as well, given how simple it is to convey and remember. These barriers include lack of resources, lack of time, lack of training to use the resources, lack of access to community providers among others[5]. Though Pediatricians use verbal education and handouts addressing healthy lifestyle, these traditional methods have probably not been effective in reinforcing the idea as the prevalence of obesity is all time high. Proposed intervention: 1. The main intervention of the project is “message for the children, message by the children”. Via partnership with “I am a star foundation” which is led by high school students, we will develop multimedia message(video) for promotion of 5-2-1-0 healthy habits. The children will be able to identify with the youth conveying message. We will show this message to the children and parents during their well child visits. This intervention will be followed by assessment of impact via “Healthy habit questionnaire” post the intervention. 2. Making the clinic a “5-2-1-0” friendly clinic is a major part of the second cycle of project. The clinic will be equipped with AAP tool kits, posters will be put up in the clinics and the staff including physicians will receive education to implement the tool kit. The grant fund will be utilized to make the clinics equipped with tools to implement the specific goal education. 3. The third intervention is linkage of families requiring assistance with resources via partnership with DOH and Feeding Northeast Florida. Anticipated outcomes: We aim to inculcate the healthy habits in our young target population – 2-9 years old children. We will have a documented baseline assessment of “Healthy habit questionnaire” pre intervention and will assess the improvement in the practices of healthy habits at home 1 month and 3 months after intervention.

Project Goal

The goal of this QI project is to optimize the education and counseling of the patients attending clinic regarding the 5-2-1-0 goals.

Project Objective 1

Improve the parent’s knowledge on healthy habits and identify knowledge gaps in the Pediatric clinics. We will have eighty percent of participants score greater than or equal to 3/4 goals of 5-2-10 on post-test survey by Mar 2024.

Project Objective 2

Decrease consumption of takeout food/fast food. We will decrease the consumption from baseline by at least 50 % by Mar 2024, as measured on posttest survey.

Project Objective 3

Identify the most effective method of education. We will increase change from pre-survey to post survey by 20% by Mar 2024.
