Although most individuals with a Congenital Heart Defect (CHD) require lifelong cardiology care, many adults are lost to cardiac follow-up or may experience gaps in care. These point of care tools aim to improve continuity of care between primary care and specialty cardiac care physicians, including when patients present in settings such as the emergency department or during pregnancy.

The tools provide concise, in depth information to support physicians caring for children, youth, and adults with CHD, and to improve knowledge with respect to advances in CHD care for children, youth and adults, and support physicians in their care for children, youth and adults living with a CHD.
