CIRCLE clinic was established in November of 2020 as an integrated care model child and family centered medical home located at the Sacramento County Health Center/FQHC operating within Pediatrics clinic only two 1/2 days per week with hopes to expand services

Program/Project strengths
We serve all children and families involved or at risk of being involved with child welfare in the Sacramento region and have an excellent team of providers with good communication with county based partners and child welfare. We provide family navigator resource services in addition to care coordination.
Program/Project challenges
Only operating at this time two 1/2 days per week does not allow us to expand our reach. Still working on increasing staffing and supports.
