Description of program/project
In Denver County, Denver Department of Human Services (DDHS) requires that all children that are place in out of home care and reside in the metro area with the exception of children that continue to see their medical provider from prior to placement. Initial medical examinations occur at DDHS's Family Crisis Center and that information is then related to the foster care clinic where ongoing care occurs. This care includes a fully integrated pediatric behavioral health clinician and a pediatric nurse case manager.
