This crosswalk illustrates how the National Survey of Children’s Health (NSCH), Pediatric Integrated Care Tool (PICS), and Family-Centered Care Assessment (FCCA) can be used together to assess and strengthen sub-components of the medical home model. By aligning population level data, clinical process measures, and family experience, the crosswalk provides a more complete picture of how medical homes are functioning in practice. The sub-components included reflect the Medical Home National Performance Measure (NPM) as defined by the NSCH. This resource is designed for Title V CYSHCN programs to support their work with families, partners in public health departments, clinical practices, and family-led organizations.
