View innovative and promising practices that embrace the core tenants of patient and family centered care. These practices share how they build on family strengths and promote positive parenting to support families in recovery. Each model of care is reviewed by the Maternal and Infant Health Core Planning Team and selected for replicable strategies that promote pediatric medical home approaches to care.
