As pediatricians know, the needs of children are different than adults. The pediatric medical home team, in partnership with families, is in the best position to address the unique needs of children. In addition, community partners, such as early intervention, childcare, and schools, play an important role in the lives of children and families. Partnerships among the medical home team, families, and community organizations can help meet the unique needs of children.
