Program Description
This project responds to a dire community need to improve adolescent health outcomes through receipt of preventive healthcare within medical homes. The project will serve 4,500 adolescents per year and work with three primary care practices to (a) identify teens behind in immunizations or preventive care; (b) outreach to bring adolescents in for care; and (c) collaborate with practices to implement specific medical home components for adolescent preventive visits.
Goals
The goals of the project are to: 1) Improve the system of adolescent preventive service delivery by implementing the adolescent navigator program and practice training/QI program; and 2) Improve the receipt of adolescent immunizations, preventive visits, and receipt of preventive services.
