Title A Medical Home for GLBT Youth
AAP Grant ID 1874
Project Year 2011
City & State New Brunswick, NJ
Program Name CATCH Planning
Topic Medical Home
Program Description
Gay, lesbian, bisexual and transgender youth are at significantly increased risk of physical, emotional and social problems. It has been well documented that these youth have higher than average rates of depression, suicide attempts, substance abuse, sexually transmitted diseases, school failure, family rejection and homelessness. Healthy People 2010 identified 29 objectives to reduce disparities between persons with nontraditional sexual orientations and heterosexuals. These focus areas include: improved access to care, education and community based programs, infectious diseases, sexually transmitted diseases including HIV infection, injury and violence prevention, mental health, and substance use .The AAP clinical report (2004) called for pediatricians to have an understanding of the unique experiences of GLBT youth, with the goal of promoting normal adolescent development, social and emotional well being, and physical health. However, services for GLBT youth in the community continue to be lacking. The proposed project, A Medical Home for GLBT Youth," is envisioned as a multidisciplinary program, designed to meet the needs of GLBT adolescents (ages 12-21). The goal of this project will be to plan the development of a medical home and service delivery model to meet the unique and complex needs of GLBT youth in our community and surrounding environs. "
Goals
1. To plan the development of a multidisciplinary program to meet the needs of GLBT youth seen within our clinics, and in the community.
2. To identify gaps in currently available services for GLBT youth in the community
3. Identify and eliminate barriers to care for GLBLT patients in pediatric practices; increase GBLT access to medical home.
4. To plan ways to make out multidisciplinary, subspecialty clinics more welcoming to GLBT youth.
