Program Description
Recent data from Lincoln Public Schools K-5th graders show the prevalence of obesity/overweight in K-5th graders attending schools with a high percentage of free and reduced lunch to have a linear relationship between socioeconomic status and obesity. The lower SES schools have much higher rates of obesity than the higher SES schools, the rates being higher than national rates. The Integrated Community Obesity Intervention Model addresses the intervention gap which exists in the Lincoln community as schools and healthcare providers work to provide continuity of care and provide services focusing on children in our community who are disproportionately affected by the obesity epidemic. More specifically, it will be a planning project, a collaboration between schools, healthcare, and various community partners to develop a model addressing obesity intervention needs in the Lincoln community. There will be the development of an Individualized Health Plan to be used in the school setting by school nurses, utilizing the clinical resources and community resources used by healthcare providers. Another focus is an assessment of community resources for physical activity and nutrition, BMI and health behaviors, access to medical homes, and gaps in clinical obesity intervention programs, with a plan for addressing these gaps.
