Title The Bridge Between Two Homes
AAP Grant ID 1878
Project Year 2011
City & State New York, NY
Program Name CATCH Resident
Topic Immigrant/Migrant/Refugee/Undocumented Health
Program Description
The goal of this project is to ensure medical, dental, and mental health services in a medical home to newly transplanted adolescents, specifically those from southeast Asia. It is proposed to do this by opening up a series of focus groups directed at 3 main groups of people: 1) adolescents (age 12 to 21) who have moved to the United States from southeast Asia over the last 6 months; 2) adolescents who have moved to the United States from southeast Asia more than 6 months ago; and 3) The parents of these adolescents. These focus groups will explore the specific needs of this under-served population and share information on the services currently available to them.
Goals
1. Evaluate the health care needs of the adolescent population that has recently moved to the US from Southeast Asia and ensure their access to health insurance
2. Link the target population with a medical home to allow access to medical, dental and mental health services
3. Build a positive relationship between the pediatric community and this group of adolescents and their families
