Program Description
The Children's Hospital Boston Community Asthma Initiative (CAI) provides asthma education, nurse case management, home visiting and environmental assessment in a culturally competent and linguistically appropriate manner for low-income children in Boston. While CAI has demonstrated remarkable reductions in emergency department visits (64%) and admissions (81%), access to these services is limited by lack of coordination and reimbursement. CAI proposes this new phase to focus on enhancing connections of children with asthma to their medical homes. CAI will assess health and quality of life outcomes, calculate cost effectiveness, and use this data to develop novel payment approaches to ensure access to comprehensive asthma care for all Boston children – an innovative approach that can serve as a replicable model.
The goals of the project are to: 1) Provide a comprehensive team approach to facilitate family centered, culturally and linguistically appropriate access to enhanced asthma services through a partnership within the Children’s medical home; 2) Provide, and improve access to, a comprehensive asthma program for low-income children; and 3)Work towards policy and systemic changes to ensure sustainable impact of the medical home model.
