Program Description
A system to facilitate improved health care transition from pediatric to adult medical homes will be developed for youth with special health care needs and disabilities (YSHCN/D) in San Diego and Imperial Counties. Major features of this proposed system, based on recommendations made by a stakeholder coalition during previous CATCH-funded planning activities, include development of transition process documents and a Web site as well as relevant education and training of both healthcare practitioners and YSHCN/D, families, and case managers. The Transition Summary Template" will enable pediatricians to provide pertinent information to receiving adult medicine physicians regarding transitioning YSHCN/D. This document will be piloted in two clinical settings before it is disseminated to other physicians in the community. The "Transition Checklist Plus" document for YSHCN/D, families, and case managers will provide necessary information on multiple topics regarding health care transition. Education and training will be offered to YSHCN/D, families, and case managers to introduce the Transition Checklist and assist them in understanding issues relevant to health care transition. The Web site will serve as a referral resource for individuals in need of an adult health care provider and will also include downloadable versions of the transition documents plus additional pertinent resources."
1. Produce resources for facilitation of improved pediatric to adult health care transition for YSHCN/D that address the needs identified in San Diego and Imperial Counties. 2. Educate YSHCN/D and their parents/care providers about health care transition issues and resources. 3. Familiarize pediatric and adult health care providers with transition issues for YSHCN/D.
