Program Description
Children in foster care are a population with enhanced health care needs above and beyond that of a typical child. The AAP recommends that these children receive their health care through a medical home to ensure coordination of their multiple medical, mental health, developmental, educational, dental, and other needs. In Milwaukee, all children placed into foster care receive an initial health screen within 5 days of placement at the Child Protection Center, and are then expected to establish a relationship with a medical home for their ongoing health care needs. Misconceptions about the health care needs of children in foster care by foster parents, child welfare workers, and community pediatricians, as well as lack of care coordination for these children, serve as major obstacles to timely establishment with a medical home. This proposal aims to: 1) provide education to foster parents and child welfare workers about the importance of a medical home, 2) provide education to community pediatricians about enhanced health care recommendations for children in foster care, 3) provide care coordination to ensure all Milwaukee children have a medical home while in foster care, and 4) identify and address additional barriers to establishment with a medical home.
Goals
1. To provide care coordination to ensure that, despite changes in placement, children in foster care are scheduled with a medical home provider and keep their appointments in accordance with the periodicity schedule recommended by the AAP.
2. To educate foster parents and child welfare workers about the need for children in foster care to establish with a medical home, and to educate pediatricians in our community about the enhanced health care needs of children in foster care, including the AAP-recommended increased periodicity schedule.
3. To identify additional barriers to establishing children in foster care with a medical home, as perceived by foster parents, child welfare workers, and community pediatricians.
