Program Description
Wyandotte County, Kansas, residents have received the lowest health index rating in the state of Kansas (105/105 counties). The health outcomes and health literacy of parents living in this community directly impacts the potential for positive health outcomes for their children. Parents of young children in this community face insurmountable barriers in identifying and accessing services in the community, including health services for themselves and their children.
The goal of the project is to expand the existing Connections screening process to include an optional health screening and referral component for young children, pregnant women, and their parents at highest risk for poor health outcomes.
