Program Description
The mission of Family Scholar House is to address all of the barriers that disadvantaged single parents face in realizing their post-secondary educational goals. To assist with this mission, University of Louisville Pediatrics is collaborating with Family Scholar House to provide health care services and create a medical home for low-income children. This project will provide the families of Family Scholar House with increased access to a medical home and to provide information on the medical issues affecting children. The process will begin with a medical needs survey being conducted on each child and through collaboration and coordination, workshops will be provided to the parents. Each family will also work with a case manager to document their medical data in a format that will make it easy to transport during transition phases in their life.
Goals
1. To identify and conduct a comprehensive medical needs assessment of the children at Family Scholar House
2. To provide workshops for families on access to medical care, medical issues relevant to the needs of the population and utilization of a PCP and to provide families with a way to house pertinent medical information.
3. To provide transportation for families to access medical care and workshops.
