Arizona State Immunization Information System (ASIIS)
The Arizona State Immunization Information System (ASIIS) is an immunization registry designed to capture immunization data on individuals within the state. Providers are mandated under Arizona Revised Statute (ARS) §36-135 to report all immunizations administered to children from birth to 18 years of age to the state's health department. The registry serves as a receptacle for accommodating these reported data. In this capacity, the registry then provides a valuable tool for the management and reporting of immunization information to public health professionals, private and public healthcare providers, parents, guardians and other childcare personnel.

Arkansas Immunization Information System
The Arkansas Department of Health (ADH) is developing a new IIS, named Arkansas WebIZ, to ensure that the state of Arkansas is equipped with advanced technology standards. It will replace the current Immunization Network for Children (INC) as the statewide centralized repository of immunization information in Arkansas. Arkansas WebIZ will provide advances such as bi-directional, real-time data exchange, designed to not only meet the meaningful use requirements of today, but also be scalable enough to meet future requirements as well. Arkansas WebIZ will function as the data backbone of the ADH immunization program, serve as a resource for medical providers, and support public health preparedness activities.

California Statewide Immunization Information System
The goal of SIIS is to improve immunization rates for all California children through an innovative public-private partnership. SIIS is a collaborative effort involving local health departments, the California Department of Health Services Immunization Branch, and a spectrum of key stakeholders across the state. SIIS is working to develop an integrated, statewide computerized registry to network each child's full immunization history. This system will ensure that health care providers have rapid access to complete and up-to-date immunization records and work to eliminate missed opportunities to immunize and unnecessary duplicate immunizations.

The State Immunization Registry is a database containing the immunization records of Delaware's children. It was first implemented in 1983 and now contains the records of close to one half million individuals. Hawaii Immunization Registry "HIR"
Since its launch in 2008 as a pilot project, HIR has grown in its number of patient records and immunizations. In September 2009, one year after the start of the pilot project, 1,847 patients and 28,944 immunizations were recorded in HIR. Today, there are over 752,000 distinct patient records and over 2.9 million immunizations entered HIR. Several factors have contributed to the large increase, including data exchange with electronic health record systems.

Almost every state in the nation has an immunization registry to help physicians and parents ensure that children are appropriately immunized and protected against vaccine-preventable diseases. In Idaho, that system is called IRIS, which stands for Immunization Reminder Information System. IRIS is a confidential, computerized system that enables healthcare providers to consolidate and maintain immunization records for the children for whom they provide care.

Massachusetts Immunization Information System (MIIS)
The Massachusetts Department of Public Health Immunization Program is committed to promoting the health of Massachusetts' citizens by reducing the burden of vaccine preventable diseases that affect the residents of the Commonwealth. As part of this effort, in 2011 the MDPH Immunization Program launched the Massachusetts Immunization Information System (MIIS), a Web-based immunization registry. The MIIS is in the process of being rolled out statewide and once fully implemented will be the official source of immunization information for Massachusetts.

The Michigan Care Improvement Registry (MCIR) is an Immunization database that tracks the immunizations given to Michiganders throughout life. It is a collaborative project through the Michigan Department of Health and Human Services. The New York State legislature passed the Immunization Registry Law which, as of January 1, 2008, requires health care providers to report all immunizations administered to persons less than 19 years of age, along with the person's immunization histories, to the New York State Department of Health using the New York State Immunization Information System (NYSIIS). The goal of the new immunization information system is to establish a complete, accurate, secure, real-time immunization medical record that is easily accessible and promotes public health by fully immunizing all individuals appropriate to age and risk. - New York Citywide Immunization Registry (CIR)
New York Citywide Immunization Registry (CIR) works to improve the immunization status of all children by consolidating immunization information. Initiated in1997 and expanding in 2005, all providers must report all immunization administered in NYC for children from birth through 18 years. The CIR accepts voluntary reporting of immunizations for those 19 and over.

The South Dakota Immunization Program aims to protect all South Dakotans against vaccine preventable disease by increasing immunization coverage levels of children and adults. Tennessee Immunization Information System (TennIIS)
The Tennessee Immunization Information System (TennIIS) is a statewide Immunization Information System (IIS) developed by the Tennessee Department of Health to be a single source of immunization records for Tennessee residents of all ages. Through TennIIS, authorized users can access the Immunization Certificate Validation Tool (ICVT) to print out validated Official Immunization Certificates for children with complete records, validation failure reports for children with incomplete records, and temporary certificates for children who are as up-to-date as possible. Vermont Immunization Registry (IMR)
The Vermont Immunization Registry (IMR) is a secure health information system that contains immunization records for persons living in Vermont. Medical providers use the IMR to find patient histories for new patients, and print IMR reports for parents who need these records for school and childcare entry. School nurses use the IMR to help find immunization records for immunization coverage reporting. Medical providers are able to use it to explore immunization coverage in their own practice and identify persons who are not up to date for immunizations. It is a valuable tool for assessing, for instance, pockets of need in the case of a disease outbreak, and it is a valuable public health tool for improving vaccination rates and reducing vaccine-preventable disease.
