Joint Principles of the Patient-Centered Medical Home

American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP), American College of Physicians (ACP), and American Osteopathic Association (AOA)

In March 2007, a consensus statement on medical home principles was developed and jointly endorsed by ACP, AAFP, AOA, and AAP. Understanding the unique needs of children and families, the Academy wishes to highlight certain critical pediatric medical home principles: family-centered partnership, community-based system, transitions and value.
