Only 43% of children and youth with special health care needs in the United States receive high quality primary care. The National Resource Center for Patient/Family-Centered Medical Home is working to make that number 100%.

The National Resource Center for Patient/Family-Centered Medical Home (NRC-PFCMH) is a cooperative agreement between the Maternal and Child Health Bureau and the American Academy of Pediatrics. With over 15 years of experience, the NRC-PFCMH is focused on ensuring that all children and youth receive care within, and have access to, a medical home.
