About the National Resource Center for Patient/Family-Centered Medical Home

• As the recipient of the National Resource Center for Patient/Family-Centered Medical Home (NRC-PFCMH), the American Academy of Pediatrics (AAP) is one of three organizations funded via this new cooperative agreement from the Maternal and Child Health Bureau (MCHB) of the Health Resources and Services Administration (HRSA). The goal of the cooperative agreement is to implement innovative programs and activities to address fragmented systems of care for children and youth with special health care needs (CYSHCN).
• The AAP is joined by Catalyst Center (Boston University) and Got Transition (The National Alliance to Advance Adolescent Health) to form a national network of technical assistance resource centers, Advancing Systems of Services for CYSHCN Network (Network). Together—in collaboration with the MCHB—these organizations will develop shared goals and activities to address three main areas to improve outcomes for CYSHCN and their families—health care financing, transitions, and patient/family-centered medical home.
• The goal of the network is to engage 90 percent or more of state Maternal and Child Health Title V / CYSHCN programs in technical assistance, training, education, and partnership building activities designed to demonstrate improvement in one or more of the following areas:
• coordinated, ongoing comprehensive care within a medical home for CYSHCN
• youth with special health care needs receive the services necessary to make transitions to adult health care
• adequate private and/or public insurance to pay for needed services for families of CYSHCN

• The goal of the National Resource Center for Patient/Family-Centered Medical Home (NRC-PFCMH) is to improve the health and well-being of, and strengthen the system of services for, children and youth with special health care needs (CYSHCN) and their families by providing technical assistance and supporting implementation of the patient/family-centered medical home model.
• The Healthy Tomorrows Resource Center (HTRC), which provides technical assistance to Healthy Tomorrows Partnership for Children Program (HTPCP) grantees, will also be administered via this new cooperative agreement. The newly funded HTRC will continue to provide technical assistance to HTPCP grantees and collaborate closely with the NRC-PFCMH to support and promote implementation of patient/family-centered medical home through Healthy Tomorrows programs, which a focus on marginalized communities.

• Over 15 years of experience with pediatric medical home implementation.
• More than 4,200 technical assistance requests answered from all 50 states, the District of Columbia, and US Territories.
• Multiple educational resources created including webinars, resource guides, monographs, and instructional videos.
• Collaboration with over 50 national, state, and local organizations.
• Distribution of medical home evidence, literature, and best practices through publication and presentation at national conferences and in peer-reviewed journals.
• In addition to the above initiatives, the National Resource Center receives funding to support the Early Hearing Detection and Intervention Program (EHDI). Support of the EHDI program allows the National Resource Center to expand its initiatives related to pediatric medical home implementation.
