"Hospital to Medical Home" is a Healthy Tomorrows Partnership for Children Program (HTPCP) located in Toledo, Ohio. The program's director recently shared the clinic's strategies and successes in measuring the impact of the project's activities with the National Center for Medical Home Implementation (National Center).

- Advancing the Medical Home Model in Pediatrics: Ohio State Innovation Model Testing Grant
