State Pediatric Medical Home Activities
Current Activities
- Alliance for Innovation on Maternal and Child Health Learning Collaborative
Pennsylvania is participating in the Maternal and Child Health Bureau funded Alliance for Innovation on Maternal and Child Health Learning Collaborative. As part of this initiative, the American Academy of Pediatrics (AAP) gathered information related to access to care and coverage issues from patient/family and clinician perspectives in Pennsylvania. In addition, the AAP reviewed current Pennsylvania Early Periodic Screening, Diagnostic, and Treatment requirements to compare the services offered with the services recommended within the Bright Futures Guidelines for Health Supervision. - Pennsylvania Medical Home Program
In partnership with Pennsylvania Chapter of the American Academy of Pediatrics, this program assists pediatric practices with implementation of care coordination, particularly for children and youth with special health care needs.- Pennsylvania Medical Home Transition Program​
This program provides support and technical assistance to adult and pediatric primary care providers with transitions of care, particularly for children and youth with special health care needs.

- Pennsylvania Chronic Care Initiative
This program involves over 170 primary care practices enrolled in regional learning collaboratives to enhance care coordination, case management, and population health outcomes. Partnering organizations include the Pennsylvania Chapter of the American Academy of Pediatrics.

- Children's Health Insurance Program Reauthorization Act Quality Demonstration Grants
Pennsylvania was a lead on one of 10 grants funded to improve health care quality and delivery systems for children enrolled in Medicaid and the Children's Health Insurance Program (CHIP).
