- Advancing the Medical Home Model in Pediatrics: Tennessee State Innovation Model Testing Grant
In 2014 Tennessee was awarded a State Innovation Model Testing grant. The grant includes a patient-centered medical home program with a focus on quality improvement in pediatric medical home implementation. A statewide collaborative that focuses on quality improvement efforts to enhance health outcomes in children and youth. The initiative standardizes quality metrics and enhances collaborative learning among participating practices. - Tennessee Medical Home Project
In partnership with the Tennessee Chapter of the American Academy of Pediatrics, this project provides tools, resources, and training opportunities for pediatric clinicians and practices related to medical home implementation.
