"Transition Health Care for Children and Youth with Special Health Care Needs" is a Healthy Tomorrows Partnership for Children Program located in Houston, Texas. The program's medical director shared details about how the program facilitates successful transition from pediatric to adult health care in community pediatric practices. - Blue Cross Blue Shield of Texas Offers New, Coordinated Health Care Plan for Children and Youth with Special Healthcare Needs and Families
Starting November 2016, pediatric Medicaid beneficiaries in Texas will be eligible to receive coordinated medical, behavioral, and social services through the STAR Kids health plan.

Current Activities
- Texas Medical Home Work Group at the Texas Department of State Health Services
This statewide work group is led by the Title V Children with Special Health Care Needs Services Program. The work group meets on a quarterly basis via teleconference and supports the vision that all children and youth in Texas, including children with special health care needs, will have a medical home that provides accessible, family-centered, continuous, comprehensive, coordinated, compassionate, and culturally effective services. - Texas Primary Care and Health Home Summit
An annual statewide conference focuses specifically on expanding access to medical home for adults and children in Texas.

- Statewide Association for Regional Medical Home AdvanCement
A 3-year projected funded by the Maternal and Child Health Bureau with the goal of building a statewide infrastructure for maternal and child health, including access to coordinated, ongoing, comprehensive care within a medical home. - Project DOCC of Houston
Developed by family advocates, this project provided pediatric resident physicians with comprehensive understanding of challenges and barriers faced by families in caring for children and youth with special health care needs.
