This change package is designed for clinicians and clinical care teams, interested in engaging in quality improvement (QI) initiatives aimed at increasing screening and follow-up rates for adolescent substance use, depression, anxiety, and suicide risk. It includes project details, tools, and resources intended to support QI efforts. The package provides a set of materials that can be utilized by practices to implement their own QI projects with objectives similar to those of the TEAMSS initiative. Additionally, it may serve as a useful resource for TEAMSS participants who wish to address other health areas not covered during this project. While it is not intended to be comprehensive or provide step-by-step instructions, it offers a solid foundation for initiating quality improvement efforts.
