Our Commitment to High-Quality, Evidence-Based Clinical Practice Guidelines: The GRADE Approach
Beginning in 2025, all new American Academy of Pediatrics (AAP) clinical practice guidelines (CPGs) and accompanying systematic reviews will adhere to the GRADE (Grading of Recommendations Assessment, Development and Evaluation) approach. GRADE is a globally recognized, methodologically rigorous framework that strengthens the quality and credibility of clinical recommendations. It is used or endorsed by over 120 organizations worldwide, including the WHO and the CDC. This change applies specifically to CPGs; clinical guidance in Policy Statements and Clinical Reports are based on a different evidence-informed process.
Key benefits of the GRADE approach
- Transparent and systematic: Ensures consistency and clarity in how evidence is evaluated and translated into clinical guidance.
- Methodologically rigorous: Applies structured criteria to the systematic review and guideline development process, including transparent methods for assessing evidence certainty and formulating actionable recommendations.
- Patient-centered: Prioritizes outcomes that are important to patients, incorporates patient and caregiver values, and accounts for real-world practice considerations when formulating recommendations.
- Well-suited for pediatrics: Offers a structured, transparent approach to developing recommendations when high-quality evidence is limited. GRADE provides clear criteria for evaluating diverse types of evidence and assessing certainty, while accounting for contextual factors such as values and preferences, resource use, and equity. This approach is especially useful in pediatrics, where randomized control trials are often not feasible.
