Moms want to know how to keep themselves and their babies happy and healthy — that’s why they come to you for guidance. This toolkit makes it easier than ever to help connect moms to the oral health answers they need, providing healthy tips to patients and conversation tactics to providers like you.

Toolkit for Healthcare Professionals
This toolkit contains materials for both patient outreach and education around oral health as well as clinical resources to help integrate oral health into your practice. The toolkit has four waiting room posters, three infographics, one brochure, dental referral forms, a conversation guide, an oral health screener, and an integration workflow.
Most states cover dental care under Medicaid during pregnancy. If your state is among them, you'll find state-specific resources in the toolkit. If not, select "Generic" in the drop-down menu.

Prenatal Oral Health Information Form and Corresponding Conversation Guide
When patients arrive, have them complete the Oral Health Intake Form. Count their response letters to each question to determine if they answered mostly A, B or C. Use the top letter score as the starting place for oral health discussions with patients in the corresponding conversation guide.
